Coroner Identifies Victim of Deadly July 12 Crash With Fire Truck

Richland County Coroner Naida Rutherford confirmed the identity of the individual who was fatally injured in an accident that took place on Saturday, July 12, along Two Notch Road in Columbia.
The accident took place approximately at 6:50 p.m. in the 7200 section of Two Notch Road, close to where Decker Boulevard crosses through. As reported by Master Trooper Mitchell Ridgeway with South Carolina Highway Patrol (SCHP), a Columbia-Richland A fire truck was en route to a two-alarm residential blaze on Bradford Ridge Lane, traveling with flashing lights and sounding its siren while moving northbound in the southbound traffic lane on Two Notch. At this time, a 2010 Nissan vehicle going north was positioned in the median of Two Notch attempting to turn left onto a private driveway. The fire truck collided with the Nissan on the driver’s side.
The operator of the Nissan and four fire department personnel were transported to a nearby medical facility for treatment following the accident.
A woman named Carolyn V Collins, who was 64 years old and from Columbia, passed away due to injuries sustained in the accident.
The event is still being looked into by the SCHP, the Richland County Coroner's Office, and the Columbia-Richland Fire Department will carry out an internal review.
